Urgent Care: Visit Overview

What to Bring

 

•  Driving license or photo ID

•  Insurance card (if you have insurance)

•  Credit card or cash for the co-pay or visit fee; we do not take checks

•  A list of your medications or the medications themselves

•  Minors require the written consent of a legal guardian prior to evaluation

Sign-In

On arriving at the Clinic please sign-in at the front desk. New patients will then be asked to review our Privacy Practices and to complete the three forms below. Existing patients who have not been seen at the clinic for over one year, or who have new insurance or a new address, will also be required to complete these forms.

  1. Patient Registration Form
  2. Protected Health Information Form. This form provides us with your authorization as to how we may contact you with any relevant follow-up information (such as laboratory results)
  3. Acknowledgment of Receipt of Privacy Practices Form (Notice of Privacy Practices)

You may print out these forms and bring in your completed forms if you wish to save time on your arrival at the clinic. If you have questions about how to complete the forms please ask us when you arrive at the Clinic.

After completing the forms our receptionist will:

 

•  Photocopy your insurance card;

•  Photocopy your driving license; and,

•  Collect your urgent care co-pay

What to Expect at your Visit

Following sign-in your visit will consist of:

  1. Triage. An enquiry into the reason for your visit, review of your medical history and assessment of your vital signs (blood pressure, heart rate etc.)
  2. Physician Evaluation. Carried out by one of our Board Certified Physicians or our nurse practioner.
  3. Discharge Instructions. You will be provided with information noting the findings from your visit, the physician’s treatment recommendations, and follow-up instructions. You will receive a copy of these instructions to show to your primary care physician. You will receive prescriptions for any medications prescribed on discharge.
